Regulatory Licensing Application Packet Requirements

Click on the link associated with each required document to access the form. After completion, please save these documents to be uploaded in the Customer Self-Service Portal during the final step of the application process.

  • State of Colorado Form DR8442 - Permit Application and Report of Changes
  • "Before" Floor Plan - Must be submitted on 8-1/2" x 11" paper and must include all items referred to in the "BLA Rules of Procedure Section 3-3" Clearly mark the area(s) that are currently licensed.
  • "After" Floor Plan - Must be submitted on 8-1/2" x 11" paper and must include all items referred to in the "BLA Rules of Procedure Section 3-3" Clearly mark the area(s) that you want to have licensed.
  • Proof of Possession
    • For Private Property: include a signed permission letter from the property owner and any required lease amendments for the modification
    • For City Owned Public Property: Include a signed permission letter from Planning and Development Services (this can be requested through an online form on the Outdoor Dining Pilot Program website)
  • Receipt for payment of State of Colorado Liquor Enforcement Division fees paid through their online portal

Apply & Pay Online

Log into your City of Boulder Customer Self-Service Account and create a request for your Liquor License for a Modification of Premises. You will be prompted to upload the supporting documents and then pay applicable city fees.

Applications are assigned to Regulatory Licensing staff on Tuesday mornings.

Licensing Process

  • Temporary Modifications require that the premise be posted for a 30‐day period, but a public hearing is only required if negative comments are received during the 30‐day posting period. If no negative comments are received, then Licensing staff may administratively approve Temporary Modifications for the City of Boulder without a public hearing and submit it to the state licensing authority for final review.
  • Once Licensing staff receives an approval from the State of Colorado Liquor Enforcement Division, a copy of your new license will be sent to the main contact email address on file.
