Regulatory Licensing Application Packet Requirements

Below is the list of required forms and documents to submit a legally complete application packet. Forms with links will need to be completed and a pdf of the form will be emailed to you, please save these forms. Items without a link are attachments that you will create and provide. Once you have compiled the list of required documents, they will be uploaded in the Customer Self-Service Portal during the final step of the application process.

  • City Report of Changes Form (all)
  • Business Entity name change supporting documents:
    • For limited liability companies, or partnerships, applicant must submit filed certificate of amendment from the Colorado Secretary of State’s website. If you cannot produce the certificate of amendment from the secretary of state showing that the licensee business entity is still the same, only the name has changed, please contact us to of discuss
    • Please contact City of Boulder Business/Sales Tax to update your account
  • Trade name changes (dba), the city form is all that is needed, and no secretary of state amendment documents are needed.
  • Officers, Directors, Partners, Members, Financiers, Primary Caregivers, Named Persons
    • Corporation, Limited Liability Company, or Partnership documents- the submitted documents should fully explain the reported change in ownership, officers, directors, members, managing members, and partners i.e. stock or share purchase agreements signed by both seller and buyer, corporate minutes signed by corporate secretary, signed letters of resignation, signed letters of appointment, signed amendments to bylaws, corporate articles, LLC operating agreements and exhibits, and partnership agreements
    • City Background Check form- this form should be fully completed by new named person to be added, including any prior violations of law by the new business manager. Please include court documents, explanations, or evidence of rehabilitation if applicable.
    • State issued ID or Driver's License copy from new persons- as described on the city background check form, please include a copy of the officer, owner, or entity manager's picture ID.
    • Fingerprinting for any new individuals being added (see fingerprinting section below)
    • Please Note: Ownership, entity manager, and officer changes must be filed at least 30 days before they become effective. Unlike business manager changes who are only employees, the proposed new named persons may not act in an owner capacity (on-premise alone, set alarm, open up or close up, supervise regular employees through work direction) until they are finally approved by city licensing. The City of Boulder also conducts background reviews on all financiers.
  • Individual Owners or Ownership Percentages
    • City Background Check form- this form should be fully completed by new named person to be added, including any prior violations of law by the new business manager. Please include court documents, explanations, or evidence of rehabilitation if applicable.
    • State issued ID or Driver's License copy from new persons- as described on the city background check form, please include a copy of the officer, owner, or entity manager's picture ID.
    • Fingerprinting for any new individuals being added (see fingerprinting section below)

Fingerprinting Completed

  • Person required to be fingerprinted as part of the Marijuana Licensing process should do so at their earliest possible convenience
    • All Owners, Officers, Directors, Partners, Managing Members, Financiers, Primary Caregivers, for persons with Ownership of 10% or More, and for Agents who manage, advise, or are paid more than $1,000 a year from the Business Applicant. Evidence of Rehabilitation or Court Documents for Disposition if applicable as specified in B.R.C.
  • Receipts for fingerprinting are saved and submitted with the application documents

  • City licensing will not review to approve or deny the background checks for the new persons until all documents, disclosures, court documents, and results from both the CBI and FBI are received by us which can take several months. The City of Boulder's background check review requirements are more strict than the State MED so obtaining a "key badge" from the state may not be used in place of city approval letter for Boulder owners, officers, and managers. It is a negotiated matter between buyer and seller whether the purchase agreement is funded before final approval by city licensing.

Apply & Pay Online

Log into your City of Boulder Customer Self-Service Account and create a request for your Marijuana Business Report of Changes. You will be prompted to upload the supporting documents and then pay applicable city fees.

Applications are assigned to Regulatory Licensing staff on Tuesday mornings.

Licensing Process

  • City Staff will begin processing your application and materials, including your background check results. You will receive an Application Review Sheet.
  • Please ensure that you forward all documentation you receive from the State of Colorado Marijuana Enforcement Division to your assigned staff member.
  • Once approved, your City Application Acknowledgment and Approval will be sent to the main email address on file.
