Regulatory Licensing Application Packet Requirements

Below is the list of required forms and documents to submit a legally complete application packet. Forms with links will need to be completed and a pdf of the form will be emailed to you, please save these forms. Items without a link are attachments that you will create and provide. Once you have compiled the list of required documents, they will be uploaded in the Customer Self-Service Portal during the final step of the application process.

  • The State Renewal Application DR8400 and Tax Check Authorization Form will be sent to you by mail by the State of Colorado Liquor Enforcement Division (LED) about 90 days prior to the expiration of the license. The reminder from Licensing Staff will include the City of Boulder renewal form.
  • Payment of State Renewal Fees must be paid on the State of Colorado Payment Portal and a receipt retained and submitted with your renewal application to the City of Boulder.
  • City of Boulder Employee Training Form completed and saved for submission with copies of all employee training certificates - It is a city condition of holding a liquor license that Licensees train all employees who serve or sell alcohol within 2 months of the location opening or within 2 months of the employee's hiring, and that the servers are re-trained to stay certified. The city can only accept trainings that are approved by the State of Colorado (e-training is not accepted).
  • A Plan for Training Compliance Form must be completed if training conditions were not met in the past. Licensing Staff will reach out if this form is required.
  • Certificate of Good Standing from the Colorado Secretary of State's website

Apply & Pay Online

Log into your Customer Self-Service Portal Account and create a renewal type permit and pay the applicable fee. All forms are submitted to the City of Boulder, once local approval is given, the city will send the paperwork to the State of Colorado to continue processing.

State of Colorado fees are paid through the PayPort Online Service and a copy of the receipt will need to be uploaded with your forms to the City of Boulder.

Licensing Process

  • The above listed items must be received by the City of Boulder at least 45 days prior to license expiration or the Licensee may face either a temporary license suspension or a Beverage Licensing Authority (BLA) renewal hearing. Renewal applications received after the license expiration date will require a $500 late filing fee to both the City of Boulder and State of Colorado and may result in non-renewal, thus requiring a new liquor license application.
  • If the renewal can be processed administratively, once a legally complete application is received, Licensing staff will provide local approval and send the application to the State of Colorado Liquor Enforcement Division for state approval.
  • Once this is received back from the state, Licensing Staff will update the license record and email the renewed licenses to the main contact email address on file.
