Below is the list of required forms and documents to submit a legally complete application packet. Forms with links will need to be completed and a pdf of the form will be emailed to you, please save these forms. Items without a link are attachments that you will create and provide. Once you have compiled the list of required documents, they will be uploaded in the Customer Self-Service Portal during the final step of the application process.
- The State Renewal Application DR8400 and Tax Check Authorization Form will be sent to you by mail by the State of Colorado Liquor Enforcement Division (LED) about 90 days prior to the expiration of the license. The reminder from Licensing Staff will include the City of Boulder renewal form.
- Payment of State Renewal Fees must be paid on the State of Colorado Payment Portal and a receipt retained and submitted with your renewal application to the City of Boulder.
- City of Boulder Employee Training Form completed and saved for submission with copies of all employee training certificates - It is a city condition of holding a liquor license that Licensees train all employees who serve or sell alcohol within 2 months of the location opening or within 2 months of the employee's hiring, and that the servers are re-trained to stay certified. The city can only accept trainings that are approved by the State of Colorado (e-training is not accepted).
- A Plan for Training Compliance Form must be completed if training conditions were not met in the past. Licensing Staff will reach out if this form is required.
